Pick the consultant you need and a time that suits you. What you send is a request: the clinic checks the doctor’s list and comes back to you on phone or WhatsApp to confirm it. Nothing is held until then.

What we do with the details you send with a request

What happens to these details
- What we collect. Your name, your mobile number, the doctor and the time you choose, and the optional note about timing. Nothing else. Please do not use this form for symptoms, test results or any other medical detail.
- What it is used for. Arranging and confirming this one appointment, and nothing else. Not for marketing, not for a mailing list, and not sold or given to anyone outside the clinic.
- Who sees it. The clinic’s reception staff and the doctor you have asked for. Your name, mobile number, the doctor and the time you asked for are also sent to the clinic’s internal WhatsApp staff group, so that reception can ring you back. Your note is not sent there, and nothing medical ever is.
- If the appointment is for a child. Anyone under 18 must have this request made for them by their parent or guardian, who gives the permission on the form on the child’s behalf. We do not use a child’s details for tracking or advertising.
- Changing your mind. You can withdraw this permission at any time, and doing so is as easy as giving it — one call to the clinic on +91 88614 99206, or one line to grievance@apexmultispecialtyclinic.com. No form, and no reason needed. We then stop using these details to arrange the appointment and delete them, unless a law requires us to keep them.
- Questions, or a complaint. Write to grievance@apexmultispecialtyclinic.com and we reply within 30 days. If our answer does not satisfy you, you have the right to complain to the Data Protection Board of India.

The front desk can arrange an appointment on the phone or over WhatsApp, and can tell you what a particular consultation involves. Please do not send medical details, reports or scan images over WhatsApp — bring them with you instead.
